Nothing new on the ultrasound. Just the same old junk - cysts, mass - but they look the same as before. No ascites, either. Interesting as we were initially told that the ascites remain unless it is being treated. She has a cold and a pretty raw nose from blowing it so much, but no tummy pain. She is having diarrhea, but that's not too huge of a deal for her. We do not have to take her in unless her fever gets to 101. Let's hope that doesn't happen. Thank you for keeping her in your prayers.

Dr. Deutsch ordered an ultrasound of her abdomen. She'll have that Wednesday morning. Her fever is on-and-off and we are just to watch it. That's all I know for now. Please pray that her fever stays away and she doesn't worsen.

We woke up around 9:30 only to have Shelby's first words be "Mom, my tummy hurts." She is just laying around playing on her DS, but when I came out of the shower she was all curled up in the chair. She is now in my bed w/her DS. She has a temp of 100 and an appointment with her GI, Dr. Deutsch, at 1:30 today. Will she be admitted? I'm taking my bags just in case...

In the ER. Had Xmas @ my Mom's today. For about 3-4 days Shelby's tummy has been giving her trouble. Today she had a temp of 101. She just stayed in my parent's bedroom the whole xmas get-together. She ate - that was good. We got ready to head home and her temp was back. We called the on-call GI and she said to get her looked at. Blood, pee and poop are all being checked. She isn't miserable (thank God), but isn't "right." Avery did puke yesterday morning and not feel well most of the day, but I don't think this has anything to do with it. I'll update when we know test results, but the GI said on the phone we're probably going to keep her on IV antibiotics until we know what or what is not going on. Just the way we wanted to spend xmas break ;-(

1st Make-A-Wish Update
So, high hopes I had of updating every day! Here I go bringing you up-to-date...Scott and I finally went to sleep around Midnight and then woke up about 2:30 a.m. Friday. The limo was there promptly at 4 a.m. The girls were psyched! They chit-chatted, giggled and were just generally thrilled to be in that limosine! I was tired and so was Daddy, but the girls weren't having it. We moved well through the airport despite forgetting a bag (with B.O.B. in it!) at the curb. The bag was retrived and we checked in and made it through security quite smoothly. Shelby was a bit afraid to pass through the "x-ray" machine, but went willingly once she saw me and Avery do it. We did get "hassled" quite a bit wearing so many Green Bay Packers sweatshirts. My flying (I've never flown before) nerves kicked in once we were waiting to board. Shelby was a bit nervous when we got on the plane - too many people and too close together for her - but calmed down once the plane started moving. Of course, that's about the time I closed my eyes and starting squeezing her little hand. She was a trooper letting me squeeze so hard and for so long. Avery (thank goodness) decided at that point to pass out. Literally, slumped over in the chair - FOR THE ENTIRE FLIGHT! How lucky is that?!?! We can't be that lucky on the way home...Anyway, landing in Orlando was much better than taking off from Midway (in my opinion), but the entire ride was too bumpy for my liking. Yuck - not a big flying fan. We made it through the Orlando airport pretty good, except for Avery starting to meltdown. When we were getting our luggage, she decided to play on another conveyor belt - thank goodness it wasn't running, but she did exert much of her 2 1/2-year-old "independence" when we pulled her off. The ride to Give The Kids The World (GTKTW) went fine and we got checked in to our villa. We played on the huge Candyland playground where we will participate in a life-size game tomorrow night. Tonight was Mayor Clayton's (of GTKTW) birthday party and last night there was a party, too. Every night there is something going on here! Today, we went to Sea World. Shamu's show was AMAZING! The girls went on lots of fun rides (even Avery) and kept us busy. They had so much fun! Right now they are back at our villa with Scott while I'm doing my internet stuff. No internet connection in our villa - that's probably a good thing b/c an internet vacation is not the worst thing in the world. Anyway, its off to Epcot we go tomorrow. My Aunt (and Uncle? and cousin?) will be joining us. Thank you every one for the prayers that got us down here. Shelby has been feeling fine. Enjoy the pictures!

Labs look o.k. WBC is up a bit (good); ANC fell some (bad). Just checked her temp - its normal. Looks like all is a go if we can make it through the next 6 hours.

Shelby has a temp of 100.8. Seriously. She had her 2nd H1N1 vaccination late this afternoon - could it be from that? I paged Dr. Deutsch and he called back in no time. He ordered labs stat at the clinic right by us. We're back home; Shelby's in bed. Now we wait to hear from him...the limo is supposed to arrive @ 4 a.m. Stressed? Yea, you could say that. Please, please pray.

Little Avery, Avery, Avery...
So she puked 2 nights, gave us a night off and then tonight dropped a diaper bomb. There's not enough air freshener in this house! Every time Scott sprays it she yells "P.u." Sorry, my brain is too tired to think of another way to spell it. She's been grumpy since she got out of the tub tonight and has been yelling at us. She thinks if we don't give her the answer she wants, it must be b/c we can't hear her so she YELLS it at us. No, Avery, we're not deaf; you're just not getting your way. She's busy next to me drawing - mind you, she's 2 1/2 and its 10:53 p.m. We have a problem here. Please GI bug or whatever you are - GO AWAY!!!

Today's labs
Well, Shelby's ANC crept up a bit. Its now at 570. Still too low for Scott and I to feel comfortable with sending her off to school. Her WBC actually fell slightly to 1.9. Avery puked again early this a.m. - fun! NOT! Shelby's Wish Granter, Ann, came over this afternoon and brought each girl a backpack full of goodies and Shelby her "princess" dress. We are all set! The packing is in full swing. If you come over, you are very likely to trip over a suitcase. Now just getting Scott through his final on Tuesday, me all set with work this week and getting everything lined up for next week, and, of course, keeping Shelby healthy. I'm sure Dr. Deutsch will want to give her the once-over before we leave and draw labs again. Please, please, pray for HEALTH. Thank you.

Nice to be home :-)
Yes, it is except for when your 2 1/2-year-old starts barfing in the night...then, its not so much fun. Luckily she only did it 2 times, but several towels and clothing changes were involved. This morning she woke up with a smile and said "I hunge." (That's "I'm hungry" for those of you who do not understand toddler-speak.) At this exact moment, everyone seems to be doing fine. Labs in the morning for Shelby so please pray that they look decent. Thanks.

Her labs look decent so we're outta here! Her ANC is 500 so she's not going anywhere but home and we're to follow-up with labs on Sunday. Her tummy looks fabulous! I am convinced that the fluctuation in her tummy size was her ascites, but who I am, but the Mommy who knows her best?!?!? Anywhoo....it is just about back to normal and not bothering her. We'll take that. Now to stay healthy for her trip...

Labs came back today stinky! ANC was up (good) yesterday, but has taken quite the drop (bad) today. Its 294. Also her blood sugar is high (bad). She's never had a problem with that before so they're watching it closely. It was 244 first thing this morning and 143 when they just did a finger poke. They'll re-check it again later today. Her tummy size & distention is a bit better. She weighs now 3.3 lbs less than she did Monday night, but her abdomen still measures 3 cm larger than her "norm." She is in at least until tomorrow. They'll re-draw labs then and see if they have improved. If not, she may be re-started on the IV antibiotics b/c infection can be really sneaky when you are neutropenic.
I haven't talked much about it here lately, but we are to leave in 8 days for Shelby's Make A Wish trip to Disney. Please pray that she gets "fixed up" to be able to go on her trip and be healthy!

I came in this afternoon to bring her clothes to come home. The paperwork was all set and ready. Then I helped her change her clothes. Her tummy is HUGE again! A stat ultrasound and x-ray were ordered, but I have not heard the results yet. We are in for another night.

Once all the excitement of last night calmed down, she (and I) slept like a rock. Not once did she wake. Good; she needed that. This morning her face and tummy looked a bit less swollen. It went from her weight/girth to being: Sunday 33 kg/72 cm, last night 34.2 kg/77 cm and this morning 33.9 kg/74 cm. Remember, 1 kilo=2.2 lbs. So that's been quite a flux for a little girl! Her ANC is dropping, though, so we have to wait for that to start showing improvement before she can bust out. Tomorrow morning would be the earliest, I think. Off for a super quick nap.

Ascites the same as yesterday - no increase. Labs pretty "stable," but wbc has dropped enough to make her neutropenic. Neutropenic precautions are on her door now. Miralax was given to see if some stool is "backed up," but she's been going already...and a lot. I'm not sure if that's the answer, but right now she's sleeping so we'll go with that and see what the morning brings.

I arrived at the hospital about an hour ago and noticed that Shelby looked different than when I left around 1 p.m. Her face is swollen and her tummy is HUGE. It measures 7 cm larger than yesterday and she weighs 1.2 kilos more than when we came in. Labs have been ordered stat and she'll be having an ultrasound here shortly. Crap, crap, crap....pray, pray, pray please. This is most likely ascites. I'll update when I can.

She's been started on Vanco and Zosyn. We'll be here at least 48 hours waiting on cultures. What was seen on the CT may just explain Dr. Deutsch's suspicion of IBS. The fever, though...we still don't know what's causing that. Daddy left for the night to be home with our very sad, Riley, and Avery, who could care less what's going on. Prayers for my middle and big girls that they deal with this admission as well as they can. Because of H1N1, the hospital restricts all visitors other than parents and grandparents. Riley is NOT happy about that. Prayers please, too, that Scott & I can juggle our work schedules and his school schedule. Sigh.

CT scans shows ascites, thickening of the lining of the bowel and inflammation of the mesentery. Ummm...I hope I got that right. They are deciding which IV antibiotic to put her on. I really don't know what all that means...I'll let you know when I do.

They should be coming anytime now to get her for her CT. Tylenol helped for a bit, but now she's getting miserable again. Labs show there's probably an infection. Where, though, is the question. She will be admitted after the CT results are in. Poor kid.

Late update
Is it really Wednesday already?!?! Sorry for the "update tomorrow..." and then leaving you hanging. Dr. Deutsch thinks Shelby might have IBS, and switched her medication to Bentyl - kinda the same as Levsin, but works a little differently. She had a good day yesterday, with minimal pain. We are praying for more days like that!
Everyone have a Happy Thanksgiving! We (minus Shelby) are looking forward to eating my Mom's yummy food! Shelby has already declared she doesn't like Thanksgiving b/c she doesn't want to eat the food. Poor kid! My Mom is making Shelby friendly (dairy-free) food, but the kid probably won't even try it. Scott and I are meeting with Shelby's beloved psychologist, Dr. Jill, in Chicago, on Friday to talk about how to tackle this growing problem. She really has a love/hate relationship with food and is way, way too young to have eating "problems." Wish us luck and please say a prayer that we can come up with some good solutions! Take care!

Sunday update
I think we have Shelby's abdominal pain under control, but now we need to investigate the "why." Why is this happening, what caused all this pain, why and will she continue the (now increased dose of) Levsin. Dr. Deutsch wants to see her tomorrow. I feel like this was all brought on by the medication given last week and now we are giving medication to do just the opposite - like her little intestines went in overdrive with the cramping and then didn't stop when the medication was stopped, thus leading to her being on medication to stop those cramps...but, its just going to lead her back to square one - the constipation and pain from that. I have been researching away about this. All I can find is that it can happen in (usually older) pediatric liver disease patients. Doctors can really never pinpoint a cause, nor quite find the medication to treat it. You just end up with a very miserable feeling kid until something else pushes them to transplant. Great. What a wonderful prognosis, but, of course, that's just the Dr. Mom diagnosis. I guess I'll let the real doctors have their hand at her, but I will fight and scream to get my kid out of pain!

Shelby's tummy
So I've written about Shelby being "backed-up." Her tummy problems subsided for about 4 days and then started up again. This kid was not only having on-and-off abdominal pain, but also was just looking wiped out. She went to school on Monday, but ended up in the nurse's office with tummy pain in the afternoon. I picked her up from school and took her to her GI doctor. He presribed a new medication to help her go to the bathroom and she's pretty much had pain since. Cramping, especially after she eats or takes the med. I have not given it to her this morning, but she is still not feeling well. I'm waiting on a call back from the doctor. [[sigh]]
Three weeks from today, we'll be on Shelby's Make-A-Wish trip!!! Let's hope we can get this resolved by then....

10 years ago...
I was starting to feel like a med-free labor may not be the route I was going to be taking. So far, walking, rocking and sitting in the tub were helping manage my labor. But, the nurse kept making me get out of the tub every so often so I could be hooked up to the monitors to make sure the baby was tolerating labor o.k. I was dilated to 5 cm...that's it! I had been at 1-2 cm for weeks and after 12 hours of labor (not hard, though...that's when my water broke) I was only at 5! What a rotten deal! Finally, there was just a time that I got out of the tub and didn't want to go back in. I asked for something to help with the pain. No epidural, though. I am pretty sure they gave me dilaudid for the pain. It worked. I felt all out-of-it, but was aware of things. I really could have cared less what was happening, but at one point remembered thinking "I don't want to feel this way when they hand me my baby." My dose was wearing off around 11 p.m. I did not want another one and that was just fine because I fully dilated in those 2 hours. It was an easy delivery with only 5 pushes to get her out. She was gorgeous!

Update
So, when Shelby is a teenager she is going to shoot me for writing about all this...it took 22 capfuls of Miralax and a follow-up x-ray to make sure she was "cleaned out." Twenty-two.....22!!!!!!!!! How does this happen when the kid is already on a daily maintenance does of this stuff?!?!?!? We will be monitoring her bathroom habits much closer to make sure this doesn't happen this bad again.
Today Avery will get her H1N1 vaccination. Its has been so wonderful to have her back to her old self!
No one has to go to work today and there's no school. Riley and I will be birthday party shopping, but otherwise we hope for a very relaxing day :-)

Avery update; Shelby's tummy; Riley, a decade?
So, little Miss Avery finally started acting like herself after today's nap. She has not had a fever since Sunday, but, boy-o-boy, had a rough week. Now we have a 2 1/2-year-old who is very off her schedule. We'll get her back on track soon...I hope.
Shelby started having abdominal pain yesterday at school. She is on Tamiflu and she and Riley did get their H1N1 vaccinations yesterday morning, but I wasn't convinced that either of those had any thing to do with her tummy pain. Her abdomen was very distended so today I decided to have her looked at. An abdominal x-ray was ordered before we saw Dr. Deutsch's NP and, guess what, she's "backed up" AGAIN. So we (and I mean Grammie as Scott was working and I had a meeting) got to come home and start a "clean out." Its 10:18 p.m - she's had 5 capful's of Miralax and only pooped a little. Sorry to be so graphic. Tomorrow will be a long day, too, for her as we need to do the "clean out" again since we didn't accomplish much with the 1st round. She hates "clean outs" and I don't blame her one bit! Poor kid!
Riley...Riley....Riley...will be 10 in 9 days. No way - it just can't be! My 1st baby - 10! Yikes!

On the upswing?
I hesitate to write that title, but after sleeping for 21 hours, Avery woke and stayed awake around 7:30 this morning. She is very congested and is running a really low-grade temp on-and-off, but is much better compared to yesterday. Go Away Flu!

I HATE the flu!
9:21 p.m.
When Avery was going on almost 9 hours of sleeping, only waking briefly to change positions, we decided it was time for convenient care. What a waste of time. We are concerned about dehydration. We changed her first diaper at about 6:30 p.m. and it wasn't very wet. She did test positive for Influenza type A (no surprise there!) and they said her tonsils looked questionable, but couldn't get a good culture swab. The doctor said give it until tomorrow and then she might need to be admitted for fluids. I'm thinking, why wait, but I will be patient unless she worsens in the night.
Please send prayers to my baby.

Shelby's temp keeps coming and going. She has a cough that comes and goes. She's on Day #3 of Tamiflu.
Scott seems backs to normal.

Flu update
10:44 p.m.
Shelby DOES have a temp in the 100-101 range. Thank GOD she started Tamiflu yesterday. Hopefully the fever will be the only symptom she gets! We called Chicago to just give them a heads-up and they said she *should* be o.k. since she started the antiviral so early. Please pray. Scott is feeling much better and little Miss Avery played up a storm today. She still needs to drink and eat more and has a stuffy nose and cough, but is MUCH, MUCH better than yesterday.

This flu is SCARY stuff!!!
Avery was really bad last night (we called on the on-call Ped and were debating taking her to the ER). Her fever was 104 w/tylenol! She was drooling and just staring at me, but then fell asleep in my arms and looked much calmer. She woke several times in the night crying and we kept up on the tylenol, but woke at 11 a.m. this morning...FEVER FREE. She ate some breakfast and is drinking some juice. We were really lucky to only have a about 36 hours with this stuff...but they were pretty dramatic. Now, we just pray that the fever doesn't come back as everything I've read says that's the time to worry.
Scott is still very achy and is going on 12 hours of sleeping. No high fever for him, just a low-grade one that didn't last long.
Shelby started crying when she learned she had to take Tamiflu. There has been so much talk in the school about H1N1 that she was really scared. Prayers please that no one else comes down with it!

Scott came home from work today - no fever yet, but stuffy nose, sneezing, cold. When I got home around 6 tonight, I picked up Avery and her forehead felt hot. Her temp is in the 101-102 range and is sleeping next to me right now. Please, do not let Riley & Shelby get this!

Congratulations Riley!
Riley was 1 of 39 Rockford school children awarded today for her fire prevention poster. She won 2nd place in the 4th grade category. We are so proud of her, and, by the above picture, you can tell she's pretty proud of herself, too. Better now
The meds made me feel so much better. I hit the ground running and put in 40 hours in my 25-30 hour week job. It was nice to be back at the school and interacting with my after school program.
Shelby went back to school last Friday, had her follow-up x-rays for her broken finger on Thursday (Yea! It healed fine.), and has an appointment with her local GI, Dr. Deutsch, for a hospital follow-up and labs again. We have to make sure her ANC keeps heading up.
Riley did not feel well earlier in the week, but recovered just fine. Avery has decided that she does not need to go to sleep at night, so we've been dealing with that fun in the evening.
And the BEST news is.....Scott will start working tomorrow at Rockford Gastroenterology Associates!!!! His CNA instructors called him up a few weeks ago saying that they got a call from GI Assoc. looking for a CNA and wondering who they would recommend...and they called Scott. I'm so happy for him that he is being recognized for all his hard work he is putting into his classes. For now, its a PRN (as needed) position, but hopefully will lead to a regular part-time schedule. He'll work 10-hour days possibly up to 3 times a week so that will be quite an adjustment to his homework time. I'm so proud of him!!!!! :-)

Update
Shelby's labs have improved; just a smidge, but in the right direction. Off to school she went today. She will need another test, though, because I've tested positive for H. pylori. Shelby does still have occasionally mysterious tummy pain. You can read the link to see why she needs to be tested because I'm still not feeling up to typing too much. I've started my 2-week triple-therapy (2 antibiotics & an acid-reducing medication) regimen. Let's hope I can start feeling better!

What's up with low WBC's around here?
So, as I said, I'm sick, too. I went to Immediate Care yesterday and I was dehydrated, but not enough to need IV fluids. My WBC (white blood cell count) came back low, though. Its normal for your WBC to fall when you have a virus, but mine was lower than the anticipated drop. In fact, mine is just a few point above "dangerously" low. Ah, hello, this is what happens to Shelby, not me! So, today I am headed back to my regular doctor to be seen again and have labs re-drawn. I still feel really crappy.
Shelby will have her follow-up labs later this morning at her Pediatrician's. We'll see how those turn out, too. She is still feeling great. Enjoy the pictures!

Home yesterday
Sorry to be so slow, but I hit the ground running yesterday with my after school program and then got sick last night! This is my first chance to attempt an update. Shelby was discharged yesterday morning with orders to stay at home and get labs repeated in 3-4 days. We have to see an increased in her ANC before she can head back to school. She is feeling great and bouncing off the walls. She declares now "I love school! I don't want to miss it!" I'll update later in the week on her labs.

Sunday update
Oh the drama today has brought thus far! It started out this morning about much concern over her labs. Long story short, it looked as if she needed a blood transfusion. Question of the accuracy of the labs was brought up, so a new set was drawn (like the 4th? poke in 2 days, not including the IV) and came back more like Shelby's "normal." Phew!
Then she was moving around in bed and cried out that her IV hurt. When she was hooked up again for her meds, she cried that it hurt and the IV site started to swell. That IV was a goner. But, because her white blood cell count is so low, fevers can not show up even when there is an infection. So, the IV was removed, but a new one had to be put in to continue the meds so she gets 48 full hours of medication while we wait on the cultures. Sigh. What an ordeal! Her veins are starting to show some wear-and-tear from being poked so many times over the years. Finally, the second site they tried worked and the meds are running again.Riley is here for the afternoon and I thought it would be helpful - as in she plays with her sister and I do some work. WRONG! She's as high-maintenance as Shelby today! I just ate some lunch (its 3 p.m.) and think I have them settled enough to attempt some work after this update.
Shelby is in here until her ANC (germ-fighting off ability) is stable or heading back up. Right now its in the 400-range = not good. She's on neutropenic precautions (again) and is having the oh-so-not-fun IV antibiotic diarrhea. If you saw her, though, you'd never guess.

Club Med Update

Guess where we are?
12:44 a.m.
Getting settled in our room @ Club Med RMH. Yup, she was admitted. What a hellish wait in the ER. Not a visit that will get high rankings from me when they send that little survey to us in the mail. Grrrr.....From what they've told me, her labs look o.k., but they want to keep her on IV antibiotics until her cultures come back...so that means we're probably here until Monday morning. My after school program starts on Monday and I have a weekend full of work so I'm not quite sure how this will all work out....but I'm thankful we're in Rockford and not Chicago. She finally passed out a few minutes ago and I'll do the same when the nurse is done taking her history and then the doctor comes in and examines her. Sigh...what a long day.

Yup, its our 2nd trip to the RMH ER this week! Shelby came home from school not feeling well. Grammie assessed the situation as Shelby and Riley were to have a sleepover there, and I was called. I actually had a doctor's appointment after work because I've lost my voice...again! I'm fine, its just my body's reaction to allergies and/or a cold. I sound horrible, but just have some sinus congestion. Back to Shelby. By the time I got to my Mom's her temp went up even more and she did not look like herself and was just laying there. I (with my Mom as my voice) called Chicago and they said get her looked at. We're taking a gamble with coming to RMH because if she needs to be admitted, they may not accept her. It might be another ambulance transport to Chicago. We're hoping we get lucky (I know that's warped thinking) and maybe she has strep throat or something. Sure would be a liver infection. Updates to come...
By the way, I've been swamped with work so I didn't update on her finger like I said I would. Its broken. She snapped that little bone all the way through. We're lucky the bone didn't displace. Two more weeks of no gym and a splint. She's not a happy camper, especially with where we are right now.

Avery the Bag Lady and Shelby Fracture #8?
This morning Scott was folding clothes and Avery came up to him and insisted he help her put on her green pj shorts even thought she was already fully dressed. Here is the final product. She had tons of toys on her doll stroller and she just kept adding things as she found them. She stayed quite occupied for a long time.
This afternoon we headed up to my parent's house to help put away tables and chairs from their birthday party yesterday. We probably weren't even there 20 minutes when Shelby was climbing up on Grammie & Papa's bed to watch TV and bent her finger back and she and Riley heard a loud crack...and then Shelby started screaming and crying...I have never seen her in pain like that. She could move the finger but cried out that it hurt. I bet my odds on getting through the ER faster than Immediate Care (they know her in the ER) and was right. We were registered, triaged, taken back to x-ray and seen in no time. Yea for Mommy instinct! It is a "suspected fracture" and splinted up. We are to follow up with Ortho in a few days. Seriously, this kid was hardly doing anything to break a bone! She was weaned off the Fosamax 2 months ago...what does this mean for her? Who knows!?!?! I'll call Chicago in the morning.....sigh...I'll keep everyone updated.

What have we been up to?
First of all, I want to say "Sorry, Dad." Yesterday was his 75th Birthday and I did not even acknowledge it on here. I feel awful!!! I love you so much! Thank you for everything you've taught me and all the love you've given me. My daughters are so in love with their Papa and I know exactly why.
O.k., so we've been busy with me trying to get my ASAP program up and running, Scott taking Microbiology and applying for RVC's RN program, Avery being a funny 2-year-old and Riley & Shelby busy learning as 4th and 2nd graders do. Here are some recent pictures of the girls. Enjoy! Health-wise, Shelby is doing fabulous! She made it through an awful hacking-cough that her sisters shared with her and didn't spike a temp once! Whoohoo! Please pray (hard) that she does NOT have a repeat of last school year (9+ hospitalizations). THANK YOU!

Chicago Day
So...Shelby had a cardiac stress test today to get her exercise o2 sat reading. It only went down to 93 so she's good for now. Then we had liver clinic where Dr. Alonso told us that Shelby doesn't technically have the Hepatopulmonary Syndrome diagnosis until her o2 sats start falling. Good enough for me. As long as we hear tomorrow that her labs look good and, as long as she doesn't have a rough year of illness like last year, we don't have to go back for 6 months!!!!!!!!!!!!!
Whohoo! I'm beat! I decided yesterday afternoon that I don't really need my head and smacked it (again). Mild concussion is the diagnosis. Yup, I feel like an idiot. Night-night.

Look at them!
Not only are there 5 amazing little girls in these 2 photos, but there are 2 that are miracles of modern medicine. They have fought hard and look at them now! Kids are so amazing in many ways, but Annika & Shelby (especially Annika) have endured more medically than 10 of us will ever face in a life time. Just wanted to share my joy when I look at these pictures.
Now I'm off to find older ones that I can scan on my computer. We have known the Tiede's since the year Shelby was born. Moreena (other than my parents) was the first one to pop her head in Shelby's hospital room when she had her Kasai.

Make A Wish update
I realized that I left you all hanging about Shelby's wish - sorry! The wish makers came and Shelby was a bit overwhelmed. Just so many things to choose from! But, as the day went on she picked a Disney trip. Now its just up to us parents to pick out some days so they can plan the trip. She's pretty psyched - Riley, too.
Speaking of Riley, her fever went up to 104 Monday night. I took her to the Pediatrician's yesterday and he said it was *just* viral. Her throat is hurting a bit and she's developing some chest congestion, but her ear is feeling better because it really was swimmer's ear (not an ear infection). As of this morning her fever is still 101. Poor kid! I have her in "isolation" in our bedroom and she's pretty stir crazy, but I cannot expose Baby A or my o-so-medically-complicated Shelbelle to these germs. Riley and Shelby are not doing well flipping roles. Riley is not used to and does not like being the sick one and Shelby does not like having to stay away from her Riley - she is ssssoooo bored (if you ask her). Please send some prayers my Riley's way that she gets better soon. What a rotten way to spend your last week of summer vacation.

Aren't we going to a "Children's" hospital?!?!?!?
Really, this is NOT a post to give Children's Memorial a bad name, but rather my venting over some personnel there that need a BIG reminder of where they are employed. Shelby's PFT technicians sucked. I'm sorry, that's the nicest word I could use. No PFT's do not hurt. But, they are new to Shelby and she likes to ask questions until she feels comfortable to move on. I usually have a pretty good sense of when the questions are real concerns of hers and when they are just time-killers so I moderate. Today, though, there wasn't a chance for that and I didn't argue it with these people because they just didn't get it in the first place. Instead I went above them and complained. I made sure my point got across. I mean, come on, you don't say to a kid "save your tears for something that really matters." Really. Those exact words were used. I couldn't get her out of that lab fast enough. Or, "We have 5-year-olds that can do this. You're 7 so you should be able to. Come on." Yup, very sucky experience. When we left there I told her that I knew she did the best she could and I did not like or approve of some of the things that those people said to her. When I voiced my complaint I did it right in front of her to remind her that she did nothing wrong. Adults! ARGH!
Anyway, enough venting. Then we headed up to clinic and met Dr. Lestrude, the Pulmonologist, who is a combination of Jimmy Buffet with shorter hair and Paul Simon. I think that's become a game for Scott and me. We think that Lou Pinella its a combination of Dr. Superina and Dr. Whitington. I digress...so Dr. Lestrude said "blah, blah, medical jargon..." really interesting to me, but boring for you guys. Exercise 02 sats were not done due to a scheduling mishap, but Dr. Lestrude would rather Shelby have a stress test done. Hopefully that will be scheduled for September 1st when we are there for liver clinic.
We swung by the fancy-schmancy new Whole Foods on the way home and got Shelby some "Parmesan" that passed her taste buds approval when we ate supper at the Macaroni Grill. Riley & Avery went today, too, because this Mommy has been away from them way tooo much lately and didn't want to spend another day away from them. Avery was fantastic and took a super-great nap in BOB pretty much the entire time we were at CMH. Riley became increasingly quiet as the day wore on. She is already battling a sore swimmer's ear, but has now developed a fever of 101.6. She is now sleeping next to me. Poor kid!

Adding a new doc tomorrow
Tomorrow we head to Chicago (again) for more testing and to meet up with a new doctor. Shelby will have PFT's (pulmonary function testing) and see a Pulmonologist. This was all planned before the HPS was discovered, but an exercise 02 saturation test will now be done because of the new diagnosis. I will update more after her appointment. Please pray that nothing "new" is discovered and that her oxygen saturation level is GREAT. Thanks!

My Yin and My Yang
Now please don't read this and say "What about her 3rd daughter?" This is my reflection on my 2 older daughter's personalities and relationships.
So, Scott and Shelby are on their way home from Six Flags Great America. Yesterday he spent all day there with Riley only to have a "lost" car keys snafu at closing time (10 p.m.) that kept them there until 11:45 p.m. The key did drop out of Scott's pocket, but instead of it being turned in to the lost and found, the security guard had it. All's well that ends well, right? But, it made for a lllooonnnggg evening. Anyway, since I am still helping all our belongings find their new home since we moved, I ran across a paper describing the meaning of Yin-Yang. I read it and Riley and Shelby popped in my head. Totally. I mean, I've said it for a long time, but they either love or hate (not really, you know what I mean) each other. What quality one lacks, the other posesses. One has a talent, but the other wants it. One has straight hair and the other has curly hair. And they both want the opposite. It cracks me up because when we brought Shelby home from the hospital (when she was born; I know their are *several* times we've brought her home from the hospital) all I could say was that I had Riley's twin. They seemed so much alike to me. In pictures they don't look that way, but that's what my eyes saw. My Grandma was the one who said "Just you wait. They'll be different." Oh my gosh are they ever! Its amazing how they have been raised in the same home, same parents, but can have such different takes on life. I won't comment on who has some more favorable qualities over the other, or which one is talented, etc. That's not what this is about. Its about the Yin-Yang. "The outer black circle represents "everything," while the black and white shapes within the circle represent the interaction of two energies, called "yin" (black) and "yang" (white), which causes everything to happen. The shape of the yin and yang sections of the symbol, actually gives you a sense of the continual movement of these two energies." I wish I could state it eloquently and profoundly of what this symbol means to me when I look at Riley and Shelby, but I just cannot. I've tried. Hopefully, you parents out there get what I'm saying. It just amazes me.

What have we been up to?
Well, we've been trying to make this week our "summer." Scott is done with his Math class (which he aced again with an "A") and we thought no one had camp. That is until Monday morning I got a call from Camp Winnebago wondering where Shelby was. Oops! My bad. She went for the rest of the day, but had no interest in going today. I think she's done with camp for the summer. Turns out that works out well because the girls each have tickets to Six Flags Great America that they earned through reading this past school year. They a have to be used by Sunday so Shelby bailing on camp makes it a bit easier for Scott to get his individual trips in with them. Yes, they each get a special Daddy day. Yea for Twickets!
Saturday night we went to a local festival so the big girls could ride the spin-til-you-puke rides in the dark. (The lights make them extra cool.) Avery just hung out in B.O.B. like the good little girl she is. The girls had a great time!
On Sunday we took advantage of the nice weather and spent the entire day at Magic Waters, once again using tickets that the girls earned. Mommy pooped out as she is trying to not consume so much caffeine (and napped a bit) by late afternoon, but everyone else was going strong.
Avery is so funny. I just have to share a few things she did today. She went and hid in her sisters' bed and yelled "Liley! Sheshell! I hiding" then waited for them to come find her. Then, just before supper she found an old winter hat of Scott's that his Dad got him when he was working on the pipeline in Alaska. She wanted it on so I put it on her and she said "Bye, I going to the snow!" We had to convince her there was no snow outside and she was quite disappointed. Her words are really coming now and she is just too funny! Man, we all have so much fun with her!
On the Make A Wish front - the wish granters are coming Saturday morning. How exciting! I think we're going to tell Shelby that she's getting a wish on Friday. I plan on taking video and sharing...watch for news!

This and that
This will just be some odds and ends and ramblings, but things that I wanted to share. First of all, if you could pray for Scott's Dad that would be very much appreciated. This morning he had surgery on his carotid artery. All went well with the surgery, but post-op his blood pressure took a nosedive so they are watching him very carefully in the Cardiac ICU tonight.
Second, I wanted to you all to see what little fish we have. Enjoy!
Third...and this one's big, but we have NOT told Shelby (or Riley) yet so mum's the word...Shelby is being granted a wish through Make a Wish!!!! I should be hearing from the wish granters soon to set up their visit!!! She sssooo deserves this (Riley, too). We are so excited! Shelby has been through so much, especially this past year, that we decided it was time to do it. Her disease is considering "life-threatening" so that makes her qualify.

FYI
Shelby and Riley DO NOT know about Shelby's new diagnosis. They don't need to. It does not affect Shelby in any way right now so we don't need them worrying about it. Just wanted to make sure that everyone understands to keep (please) quiet about this around them. As I have said before, along with the many prayers holding Shelby up, I strongly believe that her not knowing everything that's going on and wrong with her is helping her maintain such a positive outlook and a "normal" life. She does not know how sick her body is so she can't let it get to her mentally - and that's a GOOD thing!

The Game Plan (for now)
I heard from Chicago. Since it seems that Shelby's HPS is mild (because her oxygen saturation level is good) every 1-2 months she needs to have her o2 sats measured after exercising. She sees the Pulmonologist August 17th so she'll have the first test of that then along with PFT's (pulmonary function testing). She must be watched very closely now that she has the HPS diagnosis, but as of today its not leading to her being listed right now.
I have to go digest this all now. Just wanted to keep everyone updated. Please keep the prayers coming. Thank you.

Thank you Moreena - Layman's terms of HPS
I received this e-mail from my wonderful friend Moreena this morning. Thank you, again, Moreena, for taking the time to write this. As you read this, you may wonder what exception points are. When you have a secondary condition develop (HPS - Hepatopulmonary Syndrome) from your primary liver disease, its time to look at transplant, and, most likely, a person will get extra "exception" points that will put them higher on the transplant waiting list.

Here's what I remember:
In order for exception points for HPS, you need to have three things:
1--liver disease (check!)
2--positive bubble echo (check!)
3--low oxygenation
#3 is tricky, because a regular pulse oximeter reading doesn't do it. You have to have a low arterial blood gas. In order to check that, there has to be an arterial blood gas drawn, which is super-duper, extra-yucky painful (Grandparents: warm up those credit cards!). They might not want to put Shelby through an arterial blood gas draw until they start seeing a drop in the regular thumb or index finger pulse oximeter readings, but maybe they'll check it just to be sure. Annika's was always low, but never low enough to qualify for the diagnosis of HPS. And those draws were pretty much the worst, like 50 I.V.s being put in at once. Awful, awful, awful.
Chicago also had Annika do a nuclear test, the tagged albumin thingamabobby. This test is super-accurate for diagnosing moderate to severe HPS, and it's super easy. I don't even think she has to have an I.V. for that one (my memory is that they just use an injection, but maybe I'm misremembering). I bet they'll have Shelby do that one, too, since it's so sensitive to HPS and not at all painful.

Here's the lowdown, as I remember it, on a positive bubble echo. A positive bubble echo tells you that, for sure, something is going on. Shelby's blood isn't all taking the correct path through her heart and lungs. That is, blood comes into your heart on the right side, gets pumped into your lungs to pick up oxygen, and is then sent back to the left side of your heart, and then is pumped out of your heart to carry oxygen out to be used by your body.
A positive bubble echo tells you that, somewhere along the way, some amount of blood is going through some alternative pathway. However, it doesn't tell you where or what kind of alternative pathways are being taken and it doesn't tell you why the alternative pathways formed and it doesn't tell you how severe the problem is. When the problem is severe, though, you'd definitely know it because you'd have a lot of blood not getting properly oxygenated by the lungs, which would lead to Shelby being out of breath a lot (like when just sitting down or walking a few steps), and it would show up as lower numbers on the pulse oximeter. And she'd be needing oxygen all the time.
With HPS the problem is that the liver disease causes some of the vessels in the lungs to go wonky. The reason that they look at the heart (with the bubble echo) is that when you have problems in the lung vessels, you'll start seeing effects back at the heart, too (that is, you'll start seeing blood taking alternate routes to get around the problems in the lungs). I'm assuming that it's easier to spot these problems in the heart, rather than looking directly at the lungs themselves (the blood vessels that are going wonky could be really small). But, again, I'm just assuming that's why they look at the heart to diagnose a problem in the lungs.
In the setting of liver disease, without any underlying cardiac or pulmonary issues, it's pretty well assumed that a positive bubble echo means that the liver disease is beginning to affect the lungs. This is a big deal because compromised lung function (while usually reversible after transplant), makes the transplant that much more risky. Problems with the lungs would definitely be a reason to consider going ahead with the transplant, even if the liver is still cranking along doing its thing, despite the liver disease, because you don't want to wait until the lungs are a mess and then do a big major surgery.
For the purpose of exception points, though, HPS isn't diagnosed on the basis of a positive bubble echo alone, because of the non-specificity of a positive result.
I really don't know what Chicago will do with those results. I know with Annika, they still held off with transplant even after the positive bubble echo, and even after an abnormal lung CT, and even after the lung biopsy showed fibrosis in her lungs. She never fulfilled the criteria for HPS because her oxygenation, while somewhat lower than normal, never got low enough to fit the criteria. Her tagged albumin scan was also negative. They knew something was going on with her lungs, but they had no idea what, exactly.
With Shelby, though, they might take this as a sign that it's time to transplant. Shelby, after all, doesn't have a mess of surgical adhesions inside her abdomen making the surgeons push to hold off on surgery. For her, transplant right now might be the best option, especially after all those weird infections she was getting last winter."

Her bubble study was positive meaning she does have Hepatopulmonary Syndrome. We do not know what the next step is...we will wait to hear from Dr. Alonso (Hepatologist). Forgot
Shelby's sleep study results were great - NO apnea! Sorry, I forgot to mention that. I found out about a week ago. Two tests down and both good. Now the BIG cardiology test and appointment on Tuesday. Prayers please, cross your fingers, toes, whatever...we DO NOT want her to have a diagnosis of Hepatopulmonary Syndrome.

Scope Day

So, I was getting ready to post some pictures, but Dr. Alonso came out and said that the varices are still small and very few - YIPPEE!!!!!!!!!!!!!!!!!!!!!! I'm going to make this short so I'm ready to go back and see her when they come and get me. Later...

My Mom, Shelby and I are at Children's right now for Shelby's upper endoscopy. We find out if her very small and very few varices have changed in the past 2 years. Things are pretty hectic right now as last Thursday we moved, Friday we picked up Riley from camp then headed down to Lovington, IL for a family reunion, came home Saturday night, continued moving and are now here. Phew!!! We did manage to visit with the Tiede's on the way down. Frankie and Annika have grown so much and Annika is doing PHENOMENAL since her 3rd liver transplant in October. I'll post some pics soon of that visit.
For the next 2 weeks Riley is attending Totally Arts camp at a school in Rockford. She goes every afternoon and then they have a performance the last day. She went last year and loved it! A week from today Shelby has a Bubble Echo and sees the cardiologist to see if she has Hepatopulmonary Syndrome. Busy times...
